The first thing that you need to do, is complete the Inury report form. This should be completed and sent to the ORA office each time there is an injury during a game, practice or team event to a player that may require medical assistance. This could be a concussion, broken bone, knee or shoulder injury or any other injury which requires medical treatment. Submit the completed form to Ontario Ringette Association by fax or email within 7 days of the injury occurrence.

In the case where there is significant costs associated to the treatment of the injury over and above that provided by OHIP or your personal medical plans than you are able to claim expenses through the ORA provided insurance. The following form, Athletic Claim Report, is required to be completed in full in order to request reimbursement of medical costs from the ORA insurance provider. Please submit original documentation, as well as original invoicing. The insurer requires original documentation, or else claims will not be processed.
If you have any questions regarding either of these forms or how to go about making your claim please contact the ORA office directly and they can help with the process. Contact info can be found here.
